As a partner in your own care, we want you and your family to know your rights as well as your responsibilities.<\/p>\n<p>LAU Medical Center-Saint John\u2019s Hospital is committed to play a major role in the planning and delivering safe and effective patient care.<\/p>\n<p>You have the right to:<\/p>\n<ul>\n<li>Receive considerate, respectful, compassionate and non-discriminating care regardless of age, gender, race, national origin, religion, political views, type of insurance, sexual orientation and\/or disabilities.<\/li>\n<li>Receive care in a safe environment free from all forms of abuse, neglect and\/or harassment.<\/li>\n<li>Be informed by your doctor about your diagnosis and possible prognosis, the benefits and risks\/complications of treatment, and the expected outcome and\/or alternative of treatment.<\/li>\n<li>Be informed and sign a written consent for any non-emergency procedure.<\/li>\n<li>Expect to have a full medical and nursing assessment of your individual clinical needs upon admission.<\/li>\n<li>Know the names of physicians, nurses and other health care team involved in your care.<\/li>\n<li>Inquire about the experience and skills of the physicians, nurses and other health care team.<\/li>\n<li>Request medical information from the medical record when authorized by you.<\/li>\n<li>Obtain information about your health condition and treatment options from your treating physician.<\/li>\n<li>Keep information in your medical record confidential.<\/li>\n<li>Have privacy during examination, procedures, clinical care\/ treatment.<\/li>\n<li>Reject medical treatment unless required by law.<\/li>\n<li>Discontinue medical treatment.<\/li>\n<li>Request pastoral services or similar requests related to your spiritual and religious beliefs.<\/li>\n<li>Have your end of life wishes respected and carried out as long as it is compatible with national laws and institutional rules and regulations.<\/li>\n<li>Have visitors during scheduled visiting hours.<\/li>\n<li>Examine and receive an explanation of the hospital bills.<\/li>\n<li>Have an interpreter when needed.<\/li>\n<li>Your pain will be addressed, assessed and managed.<\/li>\n<li>Change your physician.<\/li>\n<li>File complaints.<\/li>\n<li>Keep your family informed of your progress upon your approval.<\/li>\n<li>Refuse involvement of family members in care decisions.<\/li>\n<li>Secure your belongings and possessions.<\/li>\n<li>Request a second opinion.<\/li>\n<li>Request a transfer to another healthcare facility when medically safe to transfer.<\/li>\n<li>Know the mission statement of LAU Medical Center-Saint John\u2019s Hospital and type of services that are provided and how to access them.<\/li>\n<li>Receive emergency medical care and treatment easily and quickly.<\/li>\n<li>Expect emergency treatment to be delivered upon arrival to the Emergency Room.<\/li>\n<li>Refer to a specialist\/consultant for special care when there is a clinical need.<\/li>\n<li>Discuss and request information related to your specific drug therapy, possible adverse side effects and drug interactions, and Monitor your compliance and proper drug use, institute interventions when necessary.<\/li>\n<li>Have drug therapy monitored for safety and efficacy and to make reasonable efforts to detect and prevent drug allergies, adverse reactions or contraindications.<\/li>\n<li>Provide appropriate security for children, disabled, elderly or vulnerable patients.<\/li>\n<li>Visual and clear signage to enable you and your family to find the way in and out of the hospital and clinics.<\/li>\n<li>Be informed about the date of your discharge in advance and be given details of all aftercare instructions.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>It is your responsibility to:<\/p>\n<ul>\n<li>Inform the hospital as soon as possible if you feel your rights have been violated or your safety is a concern.<\/li>\n<li>Protect and safeguard your property in your possession and control.<\/li>\n<li>Provide full and accurate information about your past and present health history to the health care providers.<\/li>\n<li>Comply with all requested measures to prevent the spread of infectious diseases within the hospital.<\/li>\n<li>Respect the obligation that hospital staff must provide medical treatment to other patients.<\/li>\n<li>Provide accurate information necessary for healthcare providers, insurance processing, inpatient admissions, and outpatient registration.<\/li>\n<li>Ask any questions you may have concerning your hospital bills.<\/li>\n<li>Pay your hospital bills promptly.<\/li>\n<li>Bring your medical card\/ insurance card and proper identification with you every time you present to LAUMC-SJH.<\/li>\n<li>Follow the rules and regulations of LAUMC-SJH.<\/li>\n<li>Show respect and to be courteous to the staff and restraint from using abusive language and\/or display of unethical behavior to other patients, visitors or staff.<\/li>\n<li>Accept consequences of not complying with medical treatment \/instructions.<\/li>\n<li>Inform the clinic secretary if you wish to cancel or reschedule your appointment.<\/li>\n<\/ul>\n<p>Inform the nurse in charge if you have any special requirements for discharge such as transportation
